What besides PCOS would cause many follicles in the ovaries?

an ultrasound showed i had many follicles in the ovaries. i have also had very oily skin and acne since my teen years that just won’t go away. my doctor doesnt seem concerned, he doesn’t think it’s PCOS since i’m skinny and don’t have high blood pressure or diabetes or anything. so what else could it be?
there was also a little fluid in my pelvic cavity. dr said ruptured cyst?

Some women have polycystic ovaries (that is, ovaries with lots of follicles or cysts in them) without any other PCOS symptoms (like unwanted hair growth, adult acne, irregular periods, or infertility). Even though they have polycystic ovaries, these women are not considered to have PCOS or any other disease necessarily. However, it sounds like you do have some other symptoms, like acne. If your periods are irregular (which is an important symptom!), I would recommend going to a different doctor–preferably a specialist like a reproductive endocrinologist–for a second opinion and probably additional blood tests. Thin women can have PCOS. I have PCOS, and I am not overweight and my blood pressure is low. If you don’t want to get pregnant at this point, birth control pills are great at controlling acne and other PCOS symptoms, like hair growth. Good luck!
